A 35-year-old woman presented with generalised peritonitis. Her initial CECT abdomen was reported as normal. Diagnostic Laparoscopy revealed a fish bone perforating the jejunum. Laparoscopic removal and primary repair resulted in uneventful recovery. Retrospective CT review identified two fish bones, underscoring the importance of clinical judgment when imaging and examination findings conflict.
